  • Click on "Birth Details" button.
  • Click on " Apply Birth Registration" option under the Birth Details Section.
  • A new Birth Registration Form will open on your screen.
  • Select the District and Town Panchayat from the drop down.
  • Enter your Valid Mobile Number and Email ID.
  • Enter the Child Information like DOB, Gender and name.
  • Enter Parents Information like Father and Mother Name, Aadhaar Number, Address etc.
  • Once entered all the details, click on Submit button.
